In the future, humans can live extended lives thanks to the mechanical organ replacements created by "The Union". However, if you can't keep up with the payments for your organ, The Union repo men come collect it, right out from your body.
Remy (Jude Law), the best repo man in the business suffers a heart attack on an assignment and when he awakes, he discovers that he too is now a "client". Now that he is on the other side of the process, Remy no longer agrees with the "procedure". Unable to pay his own bills, Remy's own partner Jake (Forest Whitaker) is sent to collect.
